A Lufthansa 747 crashed about a minute after takeoff in Nairobi in 1975 because the leading edge slats weren't deployed. They are actuated using pnuematics, which are closed during engine start. Switches were not reset and even though the flap/slat handle was correctly positioned, the slats didn't extend and the aircraft stalled. About 60 passengers were killed.
