Now that ETOPS has been extended more and more, are there still any significant restrictions on twin jets flying over the Atlantic and the Pacific?

On which routes do quads still have an advantage?

United and CO fly the 744 ad 777 respectively from NY to HKG. Does the CO plane have to take a bit of a detour to be close enough to an airport?

UA is switching the 744 for a 772 soon. the load factor is not high enough.
