What does the airline do if you are too large to sit in a coach seat?

Well, if the seatbelt cant fit around you, you get a Belt extension. If you cant fit into the seat at all, well, maybe they try and get 2 free seats and stick the pax in there with a Belt extension as well, with armrests back, and maybe a cushion in between the 2 seats.
