I made a sporcle game for this last year : http://www.sporcle.com/games/flymia/eruoserve
Here is the list for what I found this is scheduled service:
MIA FLL MCO TPA RSW SFB ATL CLT RDU IAD BWI PHL EWR JFK BOS PIT CVG MEM DFW IAH ORD MSP PHX LAX SFO SAN PDX SEA ANC FAF LAS DEN SJU SLC DTW

Any missing? I will update the game. As for charters I cannot think of any other airports which would receive charters on a frequent basis from Europe to the US.

Also forgot to put a few which had:
BNA MSY PBI STL SJC. I would imagine there are some more.

BA used to serve MSY if I am not mistaken?

Also, VS had flights to SJU.

Correct. It didn't last long. BA began nonstop LGW-MSY service in April 1981 after the US-UK bilateral was liberalized and permitted several new US destinations.

They dropped the route 18 months later in October 1982, as part of a cost-cutting effort to stem huge losses. I think they dropped about 17 routes at that time and laid off about 7,000 staff. They used the L-1011-500 on the LGW-MSY route.

You may find my recent North Atlantic thread interesting (it contains a link to a PDF I made listing all North Atlantic flights on an arbitrary date).

Summer 2011 North Atlantic Flights Offer Data (by BrusselsSouth Apr 25 2011 in Aviation Polls)

Answer to your question can easily be extrapolated from the said PDF.

STL

Those were the days.... I remember seeing TW L1011s, 767s and 747s, plus a few NW 747s doing charters, at STL. Now the place is full of RJs. My how times have changed in 20 years!

Oakland County International Airport ( PTK ), near Detroit, had a regular charter flight, to Stuttgart, when DiamlerChrysler was around. The flight was for DiamlerChrylser employees only.

I am sure what you mean is non-stopflights as quite often in the US a direct flight is called direct if it makes one stop ie: LGA-DTW-SFO.
