Let me go down the list.

Virgin America - Burlingame, which is a couple of miles from SFO
JetBlue - Forest Hills, though I believe they are moving to Long Island City, still away from the airport
Southwest - Right at DAL
United - In the Chicago Loop, away from the airport
Spirit - Fort Lauderdale, though I am not exactly sure where
Skywest - St. George HQ is away from the airport
Air Wisconsin - HQ is the second floor of the ATW terminal building
Pinnacle - Downtown Memphis, away from MEM
Republic - I believe they are in Downtown IND, though I am not totally sure where in IND they are
Piedmont - SBY, right at the airport I believe
PSA - Dayton, I also believe they are at the airport
Mesa - Downtown PHX, away
USAirways - Downtown PHX, I believe away as well
Eagle - Same HQ complex near DFW as AA
ExpressJet - The old company was in Downtown Houston, and the former ASA was right at ATL. Not sure where the merged operation ended up
TransStates - I believe they are right at STL, or close to it

The new Expressjet has merged into the ASA HQ at ATL.

I believe they are in Tempe. It is on the opposite side of the hill from Sun Devil Stadium. You fly over the building on final for the 25L.

The headquarters for AS and QX (I believe they're still separate buildings--someone with the company correct me if I'm wrong) are away from the airport, but not by much...just a mile or so down Highway 99 from SEA.

Alaska Airlines' HQ is not on the SEA footprint, but is within walking distance, so I'd call that "at."

EDIT: I've been beaten to the punch! Stepped away for a few minutes before hitting reply.

Horizon's ops center is at the hangar at PDX, but the headquarters is across Interntional Blvd. and a block or two south from AAG headquarters in SeaTac.

I have a very strong opinion on this. While not all staff needs to be at the airport, I feel there is value in a large part of the staff that is usually involved at corporate headquarters to being near an airport. Apart from the fact that employees in the aviation business fly more often for either work or personal reasons, there are reasons to be near the operation. For example, engineering offices should be located near the maintenance operations so that engineers can be collocated with the mechanics they support and get a better idea of what is happening. Seeing the real thing is far more valuable than a picture.

For example, UA moved their engineering management from SFO airport to the Sears Tower. That made no sense in my mind.
If you have never designed an airplane part before, let the real designers do the work!

Their headquarters have always intrigued me. Inside it seems so makeshift and austere, not at all what you would imagine from a major airline. Definitely different, that's for sure.

Same here!

Are there any other airlines with such noticeable headquarters?

Rumours are the For Sale sign is about to be put up at the entrance? The place must be worth millions... Although a drop in the ocean compared to BD's current financial state.
