ETOPS: Time Vs Distance

We all know there's 120-minute and 180-minute and 207-minute ETOPS, but I've never seen the official word on what speed is to be assumed when we convert the times to distances. Single-engine cruise on a 767 is supposedly 390-400 knots, but does the assumed speed vary from day to day? How much do other twins vary from 400 knots? Do different airlines all assume the same speed from a given type?
