A proposal is out at the moment for 240 minute ETOPS, to get certifed the airframe/engine/airline combination need to be certified.
207 aqnd 240 have been proposed to allow direct routes over the north pole to India and Asia from North America, and some of the pacific routes, eg SIN to JFK.
The ETOPS times that have been approved in the past..
138 minutes (90+15%)
Proposed, not sure if appproved for use by any operator..

Boeing is trying to get around this by bringing in a new rule for long range operations (mainly for the 777ER). Airbus is not happy, if Boeing gets their way, it will eat into the 345/346 sales. The four engine aircraft still have ETOPS like issues, but far less restrictve than two engine aircraft. Boeing made similar noises when the 332 was granted 138 minute ETOPS for the north atlantic.
The number of 777 engine problems of late will may make any certification interesting.
