Ntsb Meeting On FedEX MD-10 Dec03 Accident

Some notes here from this mornings NTSB meeting. The board has not approved these findings yet (I lost my webcast connection before the meeting concluded)

Suggested probable cause:

Improper crosswind landing technique - First officer
No Flare
Not aligned with runway

Inadequate supervision - Captain, check airman

First Officer was undergoing a check due to an altitude deviation the previous month. This was her first trip sequence since the altitude deviation.

Acft touched down left gear first then rolled onto right gear. Left gear impacted at 12.5 FPS and the right gear at 14FPS. Design spec. on the gear is 10FPS at max landing weight. Right gear suffered an overload fracture and failed at touchdown but did not depart the aircraft until 14 seconds later.

After acft stopped non-rev pilot attempted to deploy slide at L1 door. Slide inflated but disappeared beneath aircraft. Investigators found both manual inflation and release handles had been pulled on the slide. FedExs door trainer
did not have a manual inflation handle and the non-rev pilot had no training on the device.

Crew & pax exited aircraft via cockpit windows. Captain thru the F/Os window and remaining 6 thru Captains window. After first three people had exited numerous items (crew bags) were thrown out of aircraft then remaining 4 people exited. The entire evac took 2min 30sec, the last 4 to get out only took 44sec. The entire evac was captured by a bystander with video camera.

Again this is preliminary due to losing my webcast connection. Report should be out next month.
