The helicopter had a tail rotor pitch change rod failure. This caused the balance weights on the tail rotor blade grips to drive the tail rotor blades to a very high angle of attack. This is turn induced a severe left yawing moment. It was a very difficult malfunction to detect by the pilots because the failure occurred in the dark and in the clounds while flying from KLAX to KSNA.
