VH-PHA (cn 1454) Owned by R J Darrow, of Gayndah, Queensland. Written-off 5 June 2002 - 58 km SSW Lake Evella, NT - conducting survey operations, but suffered engine failure and during autorotation, rotors hit a tree and the helicopter crashed. Canx 26.8.2002 as destroyed.
