68-0465 / FW (cn 3622) 122 TFW, Indiana Air National Guard, Phantom II after an accident where the pilot was incapacitated and the WSO landed from the back seat. During the night landing, the aircraft drifted off of the runway. The collapsing nosegear caused the ejection seats to fire.
