You can see the cargo door wrench laying on the floor in front of the entrance door. The sheet metal piece in front of the cargo door was usually hinged to the cargo entrance and was to protect the cargo door latch sockets from damage by the cargo loaders. The ladder is how the crew got in and out of the airplane.
