seeing clarification on twitter from multiple sources it was a HORIZON aircraft, not Alaska

isn't there that saying about airplane crashes that several things would likely have to go wrong, including failures of backups, that all lead to the incident... well, it sounds like in this case there were lots of opportunities to de-escalate the situation, and instead there were several failures. ...
