Like any door/emergency exit issue, this probably boils down to passenger capacity. More exits = more passengers that the aircraft is allowed to carry. Comes into play when an aircraft is stretched, or when an airline decides to use a high-density seating arrangement (e.g. charters).

How come V1 speeds are depicted in speedbooks as a function of TOW only, shouldn't the length of the actual runway used play a part in figuring out the appropriate V1? Do the tables just assume accelerate-stop distance = take-off distance at that particular weight?

Perhaps to make the holding bay (or whatever that wide area just before the hold point for the runway is called) bigger? Can't be a clearance-with-the-runway issue since the parallel twy for 28R is closer, and for 28L as well elsewhere, as you've noted.

An-124s have been used extensively by NATO countries to transport military cargo to conflict zones (namely Afghanistan). Whether the U.S. in particular is chartering them too I don't know, but it's a possible explanation for the Antonov's presence.

The U.S. has already done more than I expected them to, by supplying airlift to get some of those Georgians home from Iraq.
