Maybe sheer size of the domestic market and how little it takes to purchase business class/domestic first. Most allow access for transcon business/first. As for European lounges, I'd beg to differ. The two toilets have to be adjacent because combined they are accessible by disabled passengers. This is only the case on Spaceflex v1. KL cityhopper actually still operates 13 Fokker 70's, according to Wikipedia. I'm booked onto one on AMS-LHR in a few weeks. 7P (Air Panama) flies Fokker 50's on a few routes. If you're ever in that part of the world, they're an interesting little airline.
