IIRC EK was the first to install 3-4-3 Along with TG on their 772As that were delivered in 1996 0961770 (Image from 2005) It was only installed only in the 777-200 fleet, and the 777-300 fleet that was delivered between 1998-2000 ended up with normal 3-3-3, and so did every other 777 operated by TG...

The international side of FUK (Fukuoka) comes to mind; there is a subway serving the airport, but the station is on the north side of the airport, but there is no rapid transit access to/from the international terminal beyond buses.
