FedEx operates several as the MD-10. I believe one MD-10 is operated as a flying hospital, and the USAF still operates 59 as tankers. The Netherlands has a few as tankers as well.

I remember flying on a 747-136 from PIT-PHL-LHR sometime in the early 90s as a wee kid. At some point, they moved their flight to LGW and downsized to a 767-336ER. I'm not sure about the exact timeframe though.

Hawaii is a huge transient population. People (UH students, workers etc.) need to go home for the holidays. Hawaii locals need to go to Vegas for the new year. Sounds like a good use of spare capacity.

Flight time, even on the same route, will never be the same, even with the same aircraft. Many, many factors go into route planning. Some are listed above.
