The crash remains under investigation and the Navy has released few details. One line on the 28th page of the Naval Safety Center’s mishap summary for September, first reported by Navy Times, says that when the Seahawk landed on aircraft carrier’s flight deck it began vibrating. That side-to-side movement caused the helicopter’s main rotor to strike the flight deck, which in turn led to the helicopter going over the side of the ship.

Report released today. A damper hose on the helicopter rotor failed in flight, causing extreme vibration and loss of control. Hose was pinched at some point by a pry bar, which weakened the wire reinforcing strands. No fault of pilot or crew involved. The helicopter yawed violently back and forth until the rotor struck the deck.

Wow, that’s a terrible way to go. There are no silver linings in a mishap like this, but I’m thankful the air wing (CVW-9) wasn’t embarked on her when this occurred, or the loss of life could’ve been much worse.

As they say, "Sailors die in bunches."
Sucks because it seems like a Engineering Order could have avoided this, but at the end of the day "aviation, like the sea, isn't dangerous as much as its inherently unforgiving."
Doubly so for our bros and sisters that mix the two.
