CAPTCHA issue

Anyone know that, why after passing throught CAPTCHA when making a post, occasionally, instead of successfully postimg the message, it will get sent back to the reply page ans resetted what was being type as reply?

I also noticed the Captcha doesn't work, just keeps giving me endless captcha challenges when trying to flag a photo comment for this photo:

After trying 6 times I gave up and came here.

We're looking in to it. I was able to successfully report the comment. it should be gone soon.
