I'm not sure who pays for the delivery flights (although I think that, eventually, the customer does), but concerning the crews, it all depends on the habits of both the constructor and the customer. For instance, sometimes customers may wish to fly the delivery flight all the way with a crew of their own, but, if they like, I think a Boeing crew could fly the aircraft to its destination.
Concerning the B717 delivery flight, discussed in Airliner World, a Boeing crew flew about 2/3 of the sectors of the delivry flight, and from then on, Impulse pilots took over, and flew the aircraft to its destination.
