Are there any other routes to foreign countries beginning from a foreign country currently being operated by an American carrier?

UA155 and UA154 fly between Micronesia and the Marshall Islands. Although associated with the US, they're not US territories.

They haven't for quite some years anymore. Both KL and 9W serve the route now.

UA flies Palau-Philipines (ROR-MNL) and Marshall Islands-Micronesia (TKK-PNI-KSA-MAJ).

On a separate note, are the DL (legacy NW) NRT routes count as 5th freedom or are they something different dating back to post WWII?

That’s been history for a while along with Delta’s CDG-BOM and United’s HKG-DEL flights. Weird routes to India in the 2000s.

Delta's PDX-AMS route used to continue on to BOM (as PDX-AMS-BOM), but the AMS-BOM leg ended a long time ago. And as someone already mentioned, KL and 9W (which are partner airlines of Delta) currently operate the route now...
