It doesn't say a pax activated the slide. They said it was activated during the loading of inflight meals; sounds like the catering employees did open a door that didn't have its slide de-activated

The picture is from the incident at MAN. AP-BHW is a 77W and PEK-ISB is a 772.

Or, somehow, the cabin crew forgot to disarm/crosscheck that particular door. Since the aircraft was obviously between flights, the doors all ought to be disarmed. But surely that would be incredibly unlikely... you'd hope!

And there is still no mention that a passenger did open the armed door.

Hi: Double Failure will best describe this accident.
Plane parked, all Arm-disarm handle Must be positioned in “Disarm” and do not placed back to “arm” position just during a/c preparation for push back, upon crew command.
Any way, maintenance personnel are not authorized to open close doors when crew and pax are o/b, except in emergency..
For catering, without a training on the plane/ emergencies- doors, this is a no-no. They should knock the door and wait for it to be opened..
There are companies very picky on that, even firing person from job, for unadvertised deployed a raft. Good way to be caution.
Note: FAA requieres a report describing how we’ll raft deployed, since is no common to check that in real life

Hi: Don’t want to be... but just a recapitulation.
As per many companies, FA in charge of this Work Station (door and surrounding area) , I.e. the FA that will T.O. And Land sitting next to this door is responsible for the position in which all items in her station are: switches, circuit brakes, panel doors, ...and of course, for the door or emergency escape door. He/She is primarily responsible to keep Arm-Disarm handle open when plane is parked.And will receive a serious “call”
Main Purse, normally audit emergency equipment before boarding, and Do command positions verifications and position changes.
(In old dayFE, with Doors open - close and arm - disarm indications, will normally call MP and advised of discrepancy)
In serious companies, responsibility is placed over FA and a Note go to MP file. On bads, only who open the doors (who, I repeat, has also some, minor responsibility for operate a system for which he is not qualified, will be fired.

If the door is in the arm position, then opening the door from the outside automatically disarms the door, hence disarming the slide per se. Yes unless pins A and B are installed in the slide then its always active, but for the sake of conversation, this motion disarms the door/slide combo.

Early 767's had the disarm lever above a window seat and it was exposed. At the gate, a grandmother got up from her seat and used it as a grab handle. Popped the slide and cost about $10K (1984 dollars) if I recall correctly.. Happened several times until a service cover was installed. Later models redesigned and/or relocated as I recall.

Maybe I'm not picturing this right, but how would moving the arm/disarm lever "pop a slide"?

Sounds like she opened the door. Doors.
electrically go up on the 767. No doubt door/slide still armed.
