NTSB report on 2019 Hawaiian Safari helicopter tour crash

Wed May 11, 2022 4:46 am

The NTSB has released the final report on the 2019 AS350 helicopter tour crash near Kekaha. The probable cause was pilot flying into IMC conditions under VFR rules, as well as insufficient safety procedures at Safari Aviation. Other operators that day had turned back due to low visibility.

However most of the media coverage is on the lack of FAA weathercams installed in the area, which tour pilots could consult either before leaving or in transit. And inadequate FAA oversight of tour operators.

This may be a case where the NTSB is chastising the FAA for not fully implementing previous recommendations for cameras and oversight.
