OK, here is my list. Some of these are now defunct. Some I have flown lots of times and others are never to be repeated. The best has been British Airways, both short and long haul and the worst Hemus Air - a dodgy Bulgarian airline. One of the cabin crew forgot to sit down for take off as she was too busy trying to keep a cupboard door shut! It was a very rickety Tupolev 154.

Somewhere around 90/100. They, and flying, has changed so much I have no clear favorites. It once was a classier affair for less people and now it is the classless fare for the most people. Flying has gotten more in reach of more folks just as it should, but mass simply dilutes the differances. Highly industrialized nations seem to have more homogenous carriers. That makes the standouts the singular cultures. Singapore is one example.
