Are these handed out on all lhr-sfo flights, and are they reused or kept by the passenger afterwards?

Yes they should be offered on all flights and are yours to keep. Most of the time I find passengers just leave their used ones on the seat after the flight and they are then thrown away

I had the pleasure of travelling back from Mumbai in First Class a few months ago - I was asked if I wanted a 'sleeper suit' and then was handed my pj's. I've kept mine and use them at home as they are quite comfy and cosy!

I find them too warm, particularly if the cabin is overheated, as it often is. I need to be cool to really sleep well. So I take them every time they are offered but I usually give them away, although I have a fair stash of them.

Instead of the pyjamas, I bring my own loose jersey knit (i.e. t-shirt material) shorts and t-shirt and nip into the loo and change into those during boarding, then change back into my clothes at the 40 minutes to landing mark. I do this flying Club or First. I might look a sight, but I do not care: I am comfortable and get a good night's sleep.
