Do any airlines have policies against operational upgrades?

Today I randomly remembered a weird flying experience I had earlier this year. I was flying from ATL to ICN on a Delta A350, only to find out upon boarding that my assigned seat had a broken headrest which the crew was unable to fix. I informed the cabin crew about this, something which I now sorely regret as they went through all the trouble of finding a passenger to bump off the completely full flight (at least in Y) for $1000, delaying the whole departure by 30 minutes. Had I known that this would happen I would have shut up and let the bracket poke me in the back of my head for 15 hours rather than causing so much trouble. My question is: why was no frequent flyer elite upgraded to Premium Plus or Business Class instead so that I could be reseated in their seat? Is it likely that the upper cabins were completely full also, or do airlines actually have policies against operational upgrades nowadays?

Unsure of the procedure here but my guess is that on such a route as ATL-ICN, that J was full as well so there was no choice.
Were you also offered to be bumped until the next day yourself?

No, I wasn't. I don't understand why I caused so much trouble either as I was saying "if it's possible to get me a working seat, that would be great" or something along those lines; I wasn't demanding to be moved.
