For me the only airline I've ever flown that isn't anymore is Air Holland. It was the first flight I ever took, one of their brand new 757s. Interesting enough, now many years later that plane is still in service. It has been converted to a freighter and is flying for FedEx.

All other airlines I've flown are still around.

Quite sad to think about, I made it almost 100 carriers which I've flown that are no more now.

Not able to list them right now as I'm only on my mobile, but was interesting to think through.

A few Swedish domestic carriers are on my list. Picture links to aircraft types I have flown with the livery they had when I flew them. The first carrier I have flown some 50 times and on three aircraft types. All flights with these four operators were 30 to 50 years ago.... :old:

.. and one not defunct - Malmö Aviation (TF), which has turned into BRA which now uses the code TF.

A fair few regional Australian ones - JetGo, Flight West spring to mind. Ansett.
US Airways, Continental, Virgin America in the US.
Air Pacific in Asia Pacific.
Air Estonia in Europe.
Surprisingly every carrier I've ever flown in Africa is going strong, though we'll see about SAA.

I've tended to rail against these times of forums (lists of flights/airports/etc are racked up -- (we get it: you're jet-setters).. much to the boredom of most other forum readers.... but some people obviously enjoy these, so I'll bite just this once :)

I'm not gonna bore anyone with my list, but one of my most memorable airlines was Kiwi Airlines in the US. A short-lived airline in the 80's and 90's; my last flight was on the glorious 727 in 1998.

Absolutely first-class operation, incredible service, and such a shame they didn't last.

My most "obscure" entry is Brindabella (FQ) which was a small Australian regional carrier flying Metroliners and Jetstreams that was grounded by CASA over maintenance concerns in 2013.

Flew on an obscure airline called "Valuair" back in the mid/late 2000s, on an A320. If I'm not mistaken, they have been rebranded as Jetstar Asia.

Régional Compagnie Aérienne Européenne and to make it more interesting, that was a Air France flight (by Hop!, operated by Regional), now how should that not work out.

Regarding Fordair, I was unfortunately to young to remember what exact aircrafts those flights were, except one which was a chartered BAe 146. The others were T-tail, so propably the MD-87. What I do remember though, is that they had their own small terminal at Stansted with an own security checkpoint and so on. Maybe someone still remembers where that was located at Stansted? I dont.
