ACE, LPA and TFS are such examples. ACE has no footpath off the airport. TFS is in the middle of nowhere, access by car only. From LPA you can walk to Ojos de Garza if you know the way, but it is not a clear footpath.
CDT is in the middle of nowhere. You can get there on a dirt road but it is a long walk. GRO and REU have no foot access either.
Even in BCN it is not easy and you must know the way.
In Italy BGY has easy foot access to the city. MXP does not, you cannot walk off MXP.

Actually Osaka Airport (ITM) is in a densely populated area, and you CAN walk there. Kansai Airport (KIX) is in the middle of Osaka Bay.
