Hmmm.. This is a hard one. The only one I can think of is ORH (Worcester, Massachusetts).
In 2014, I went there just to rent a car. And when I was returning it, I happened to see people coming in for a B6 flight (to either MCO or FLL). I believe these were the only two commercial flights out of ORH at the time.

In that case, I suppose I could add ONT to that list. If I had time to kill, I would ride my scooter from the truck stop to the rental car facility, grab a car, and hit Santa Monica Beach for the day. Return it and then ride my scooter back to the truck stop.

MGM - picked up a cracked windshield on a rental car and had to switch cars via the Avis counter. Only made it into the car rental area and was on site for 20 minutes, max.

DSM - dropped a colleague off for his flight back to STL while we were driving across Iowa (I was driving on home to Kansas City)

Edinburgh Airport in Scotland, and Warsaw Babice in Poland are the only two.

I have driven by all the commercial airports in the greater Los Angeles area, including Palm Springs, but I have only ever been "to" LAX, sometimes just for pick-up and drop-off, sometimes to fly myself.

I am counting ONLY the airports for which I have actually been on the grounds for a specific purpose, and not just "drove by":

1) SBP - I had originally planned to fly SBP-SAN this last Thanksgiving, but AS pulled out of that market (at least temporarily). I still had to visit to pick up my rental car.

2) SGF - My trip was SAN-DFW-FSM round-trip, but a broken rental car necessitated a trip to this lovely airport while visiting family in the general area.

3) OAK - in the 90's, while visiting my sister in Davis, CA, I arrived by train, but friends of ours were arriving into Oakland. Drove and parked, picked them up, and that was that.

4) TUL - many times our family in Arkansas journeyed to Tulsa to catch a non-stop to the west coast. The only airport of this group for which I can say "I went to the gate".

So I've been to a lot of airports. It's a bit hard to remember which airports I haven't flown from. The ones that come to my mind right now are:

Duluth, MN (DLH). This was at the old terminal building, and before the new terminal building was even under construction. I remember seeing a FedEx A300 or A310, and I believe Midwest Connect was operating the short-lived DLH-MKE with Dornier 328 jets.
