UA First Class Upgrade
How difficult is it to upgrade from Economy to Business Class on UA using points? Is it a sure shot as long as a business class seat is available?

It is a sure thing if there is upgradable space available.
A few questions...
1) have you purchased the ticket already?
2) what is the fare basis?
3) what is the routing?
4) do you have elite status?

likely MFR-SFO-IAD but possibly MFR-DEN-IAD

Sfo-iAd often has 3-class internationally configured planes once daily. I would try to get that.
You should set your preferences to expert mode on Ual.com. When searching, look for the "r" bucket. "R" shows upgradable space. If "r" is greater than 0, you can confirm the upgrade right away. Depending on your fare basis, the upgrade will cost a combination of miles and money.

How exactly do you do this? Does it show the fare classes when you're searching by doing this? That would be pretty helpful if it did.

1) you have to be signed in to your mileage plus account
2) click https://www.united.com/web/en-US/apps/account/preference/searchPreference.aspx
3) check box about expert mode, save at the bottom
4) search by price or schedule, award travel won't work when just checking buckets.
5) fare class will now be hyperlinked in results, click it

As mentioned above, I'd try to confirm the R class availability before making a booking. If you book a flight where R isn't available, there's still a chance although it may be a long shot. This is a busy hub to hub route and the waitlist for upgrades can be as high as 50 or more people. If there's no R class availability and you really want first class, the only sure thing is to buy it outright...

Say I book R class and then upgrade with points, am I still eligible to earn points since I at least paid the coach fare?

R is the upgrade fare class, not one you can book into initially.
But, to go along with the basis of the question, if you fly in R, you get the points based on original purchased fare before you upgraded into R.
