Diversions VS Holding Patterns

The fog in London though not as bad as in years past is still legendary. My question is this... After a hop across the pond, what determines a diversion, besides the obvious lack of fuel as opposed to a holding pattern?
I have been in several holds above LHR but never diverted, not that a diversion is a preferred occurrence, but neither is a 50 minute hold, going around and around and nothing to see.
Can any pilots expound?

Fact is - it is passenger convenience (besides fuel) that would be the concern so sometimes, holding is the option, because many passengers connect to other flights. Oftentimes, the decision to hold or divert is made by operations (although captain has full authority to decide otherwise) by the time we are some 45 or 60 minutes from destination.

I often fly EZE to MAD, our alternate is BCN... because better convenience for passengers for connections, or ground transport. Be aware that holding is not always due to bad visibility, but to traffic...

If the weather is marginal at destination, and most airplanes try an approach but miss their landing, I generally proceed directly to the alternate, since fuel reserves, is our main concern... Trying an approach, then perform a missed approach is a lot of fuel waste... Most of the time we have about 1 hour of fuel remaining when we land at destination.
