The only ones that come immediately to mind are the Valujet 592 accident back in the 1990s (improperly labeled/shipped oxygen generators) with an in-flight fire and subsequent crash, and a more obscure one, a Delta 727-200 that burned up on the gate in
SLC back in the 1980s due to (IIRC) an oxygen leak. There was also an ATA DC-10-30 that burned up while on the remote stand at ORD, after a seat was being worked on and the oxygen generator was triggered. That one was also in the 1980s...
